The undersigned health care professional (hereafter, the "client") hereby confirms to Victoria Pharmacy Zurich that:

Lamprene is ordered for non leprosy treatment under strict indications and under medical supervision of patient.

Health care professional will carefully advise patient with detailed instructions including possible side effects of this drug and will oversee treatment.

The ordering medical doctor or pharmacist is Iicensed to practice medicine in the country, state, or other applicable jurisdiction. Lamprene™ will be used only as directed by physician and only by the person for whom it is prescribed.
MD’s / Pharmacist’s license to practice is approved by DocCHECK™ prior to order or copy of professional license to practice is sent along with order .

Due to legal regulations in Switzerland we have to require an original medical prescription issued by ordering medical doctor if medicine is sent directly to patient.

The MD or health care professional acknowledges and agrees that it is his/her responsibility to have patient regular physical examinations including all suggested testing to ensure patient can safely take the requested medications without contraindications. The MD is aware of the possible side effects of product prescribed (Lamprene™).

It is the medical doctor’s responsibility that his patient uses LAMPRENE™ under his guidance.

Victoria Pharmacy Zurich has profound experience in sending medicines all over the world, taking care of all the documents needed for prompt arrival.

• But in case of the event that the requested products are detained and confiscated by foreign customs the risk of loss or delayed delivery lies completely with the purchaser.
• Victoria Pharmacy Zurich is not responsible for returning of products caused by improper delivery due to incorrect address caused by the purchaser or other reasons not caused by supplier.
• Client understands that once shipped, due to legal regulations and finally for your own safety, no pharmaceutical product may be returned or exchanged nor will be refunded to buyer !

Victoria Pharmacy Zurich has the right to refuse shipment of medicine to patient without any further comment!

The buyer is responsible for determining his/her country’s importation regulations. Any legal action that might be brought against the purchaser, if not acting in accordance with the regulations and importation laws applied by the government of the destination country, is solely the responsibility of the purchaser.

Client agrees that any and all agreements and contracts between client and Victoria Pharmacy Zurich shall be deemed to be made in Zurich Switzerland, and accordingly shall be governed by and any dispute that may arise between client and Victoria Pharmacy Zurich shall be resolved under the laws of Switzerland. Client agrees that the courts located in Zurich Switzerland shall have sole and exclusive jurisdiction over any dispute between client and Victoria Pharmacy Zurich.

Patient is instructed that he must immediately contact his prescribing doctor or healthcare provider in case of any problem occurs related to the drugs sent to him/her or patient who is prescription for !
